Consists of giving insulin every 30 minutes based on the blood glucose readings as follows:
<150 mg/dl: 0 units of insulin
150-220 mg/dl: 2 units of insulin
201-250 mg/dl: 4 units
251-300 mg/dl: 6 units
301-350 mg/dl: 8 units
351-400 mg/dl: 10 units
> 400 mg/dl: inform the MD on call

Automated protocol consisting of an insulin infusion pump named The Space Glucose Control System.
Intervention:
For glucose measurement, a sample of blood gas will be taken every 30 minutes. Actrapid HM will be used in a 4IU/ ml concentration for infusion in a 50 ml syringe.
The range of glucose will be recorded throughout the intra-operative period. The number of hypoglycemic (<70mg/dl) and hyperglycemic (> 200mg/dl) events will be recorded.

Gemcitabine doses: 300 mg/m2/week for 6 weeks.

Cervical Cancer

Gemcitabine 300 mg/m2, prepared in 0.5 liters of sodium chloride 0.9%, IV administered in 30 minutes weekly for a maximum of 6 weeks.
Radiotherapy will start the first week, or as soon as the blood count is normal or the patient has recovered after blood transfusion. Radiotherapy will be applied, using an external beam releasing 40-50.4 Gy in 20-28 fractions: 1.8 Gy/day for 5 days/week, during 4 to 6 weeks.
Intracavitary brachytherapy will be added to reach a total EQD2 dosage (α/β=10) of 78-86 Gy.

Safety of gemcitabine regarding renal function will be assessed through glomerular filtration rate.
An improvement of 10 ml/min/1.73m2 as compared to basal glomerular rate is expected by the end of treatment.

Toxicity will be evaluated according to the toxicity criteria of CTCEA v. 4.03. A toxicity index of <4 is expected during treatment.

Phase II open, single arm, national, non-randomized clinical trial; to evaluate safety of gemcitabine in patients with locally advanced cervical cancer and renal dysfunction, referring to the INCAN.

Subjects will be randomized 1:1 to high dose vitamin D defined as 4000 IU per day for 3 months.
Subjects will be given Vitamin D3 supplementation at 4000 IU daily.

The Unit of Measure is Efficacy. The primary outcome of this study is to determine if vitamin D supplementation reduces the medication usage in subjects with CUA. Thus, for the outcome of reduction in pills, at 12 weeks, subjects whose pill usage decreases by 2 or more pills per day will be classified as improved. Subjects whose pill consumption did not change or increased will be classified as unchanged.

Medication Usage

The Unit of Measure is Efficacy. The Total Urticaria Severity Score (USS) ranges from 0 to 93, higher scores = worse symptoms. This secondary outcome of this study is to determine if high dose vitamin D supplementation improves the urticaria severity score (USS). The change in USS will be compared between the groups using the independent sample t-test (assuming the distribution is normal). Logistic regression and multiple linear regression will be used to adjust for possible confounders.
