cardio-respiratory events are defined as:
Hypoxic episodes (SatO2 < 80% for at least 4 seconds)
Apneas episodes (absence of breath for at least 20 sec or at least 5 sec if associated to hypoxia or bradycardia)
Bradycardia episodes (heart rate <80 bpm)

necessity of endotracheal intubation (FiO2 >40% and/or apnea episodes and/or increasing pCO2)

Possible indicators:
gestational age at birth birth weight delivery type APGAR at 1/5 minutes (and 10 minutes if effectuated) steroid prenatal prophylaxis surfactant administration type and duration of MV previously administered (if any) type and duration of NIV previously administered (if any) corrected gestational age at enrolling caffeine doses administered (if any) FiO2 maintained during monitoring EGA values at start and end of monitoring

Participant will receive caffeine citrate 20mg/kg IV within 2 hours of life and placebo (normal saline IV) at 12 hours of life. Both infusions will be of identical volumes and appearance, and will be administered over 30 minutes.

The intervention in this study is timing of the initial caffeine dose. In the Caffeine arm, participants will receive Caffeine Citrate 20mg/kg IV (volume dose 1ml/kg) within 2 hours of life, and Normal Saline (0.9% NaCl) 1ml/kg IV at 12 hours of life.

Each participant is assigned to one of two arms of the study.

The participant and participant's parents, medical care providers, and investigators will remain masked to the randomization.

Infants in Group A were cycled between NCPAP and nasal prongs. For the first 12 hours, infants received 10 hours of NCPAP and 2 hours of 1 litre per minute of nasal prongs (NP). For the next 12 hours, infants received 8 hours of NCPAP and 4 hours of NP. In the subsequent 24 hours, infants alternated between 6 hours of NCPAP and 6 hours of NP. In the last 24 hours of intervention they alternated between 4 hours of NCPAP and 8 hours of NP.

Infants randomized to group B received continuous NCPAP at a CPAP distending pressure of 4 cm of water for 72 hours. Both the groups after 72 hours of intervention were weaned to 1litre per minute NP. During the intervention period all infants were scored using the ACoRN respiratory score on a 12 hourly basis.
