This study will be a double blind randomized controlled parallel study

200 mg of Hydroxychloroquine orally three times daily for 10 days plus 500 mg of Nitazoxanide orally twice daily for 6 days

Standard care delivered in the COVID-19 isolation hospitals.

Both hydroxychloroquine & nitazoxanide will be administered orally to participating patients

Hydroxychloroquine plus Nitazoxanide

Oxygen administered via ventilator. In addition, antipyretic "paracetamol" may be added if necessary

Pyridostigmine in Patients With Severe Acute Respiratory Syndrome Secondary to SARS-CoV-2 Infection

Participants will receive pyridostigmine at a dose of 60 mg / day (or matching placebo), P.O. during a period of up to 14 days, until hospital discharge, death, mechanical ventilation, or increase in the SOFA scale ≥2 points.
The proposed dose is a safe dose according to the experience in myasthenia gravis and healthy people, as well as in at least three clinical studies in people living with HIV.
Participants will be double-blind 1: 1 randomized to receive pyridostigmine or placebo for up to 14 days.

Sugammadex 2 mg/Kg will be administrated as a single dose via intravenous injection upon completion of surgery and guided by peripheral nerve stimulation.
The drug is to be diluted with normal saline to make up to 5 ml volume to maintain blinding.

Neostigmine 0.040 mg/Kg, along with Atropine 0.015 mg/kg, diluted in normal saline to make up 5 ml total volume to maintain blinding.
Neostigmine 0.040 mg/Kg, along with Atropine 0.015 mg/kg will be administrated as a single dose via intravenous injection upon completion of surgery and guided by peripheral nerve stimulation for reversal of neuromuscular blockade produced during surgery.
Atropine sulfate-diphenoxylate hydrochloride combination will be an adjuvant drug to balance muscarinic side effects of Neostigmine, when Neostigmine is administered.
