A 40-year-old male presented with an 8-month history of episodic painful swelling of his tattoos, associated with irritation of his eyes and blurred vision. Each episode lasted for a few weeks, with ocular symptoms preceding the skin symptoms by up to a week. A fever, rigors and diarrhea was reported during the initial presentation but this did not recur subsequently.
